First-trimester PoCUS enables rapid bedside confirmation of intrauterine pregnancy and assessment for ectopic pregnancy, making it a critical tool in early pregnancy care.

In a patient with early pregnancy, abdominal pain, or vaginal bleeding, first-trimester PoCUS is a critical bedside tool.

Transabdominal ± transvaginal PoCUS helps clinicians rapidly:

- Confirm an intrauterine pregnancy (IUP)

- Assess for free fluid concerning for ectopic rupture

—

The 3-2-1 Rule to Confirm IUP

A structured approach to avoid false positives:

Confirm continuity

- Vesicouterine continuity

- Utero-cervical-vaginal continuity

→ Ensures the pregnancy is truly within the uterus, not cervical or adnexal.

Confirm an IUP structure

- 1. decidual reaction, 2. gestational sac with 3. yolk sac OR 3. fetal pole with cardiac activity

Confirm myometrial mantle thickness ≥ 8mm

Key principle:

Seeing a true IUP effectively rules out ectopic pregnancy, except in rare heterotopic cases.

Performed in minutes, first-trimester PoCUS can reduce diagnostic delay, guide disposition, and improve safety in early pregnancy presentations.

This is why early pregnancy PoCUS is a core skill across acute, primary, and frontline care settings.
