Lung PoCUS is a powerful bedside tool for pneumothorax detection. With proper technique, it can identify key signs such as absent lung sliding and the barcode sign—often outperforming chest X-ray.

MYTH: Lung ultrasound can’t reliably detect pneumothorax.

REALITY: Lung PoCUS is more sensitive than CXR for pneumothorax detection.

- Absence of lung sliding

- Barcode (stratosphere) sign

→ Highly specific findings for pneumothorax when used correctly

Technique matters:

Scan the most anterior chest, look for lung sliding, B-lines, lung pulse — and confirm lung point when present (in stable patients).

Clinical takeaway:

Lung PoCUS should be a first-line tool in suspected pneumothorax — especially in trauma and critically ill patients.
